The attestation of activities, filled in and ready to sign
A tab under Documents. The official form from Decision 2009/959/EU in 23 languages, pre-filled with the details of the company, the driver and his driver card. Choose the period without recording and the reason, check the preview and print the PDF to have it signed.
- The periods in which the card was out without a manual entry are ready to choose.
- Date of birth and driving licence number come from the driver card, the address from the depot.
- The text of the form is the official text in the selected language.
Please note
A member state may not make this form mandatory (Regulation 165/2014). It is recommended to accept it if nothing could be recorded on the tachograph for technical reasons.
